Fact-check: Weber State University's insulin syringe demonstration

What the video covers

A nursing instructor demonstrates insulin skills for students: the insulin syringe and its units scale, drawing air equal to the total dose, scrubbing multi-dose vial tops with alcohol for about 20 seconds, the clear-before-cloudy rule for mixing regular and NPH insulin, injecting air into each vial in the right order, drawing each insulin, a one-handed scoop recap, then the injection itself at a 45-degree angle with a quick insertion, a slow push, a brief pause and the used needle straight into a sharps container.

What it gets right

The syringe handling is textbook. Injecting air before drawing, cleaning the stopper of a multi-dose vial before each access, reading the units scale, inserting without hesitation (a slow push through skin hurts more), injecting slowly, pausing before withdrawal and not recapping a used needle are all correct and apply to any subcutaneous vial, including a GLP-1 vial. The instructor's insistence on cleaning the site and letting it air-dry, and on washing hands and gloving in a clinical setting, is standard.

What it leaves out or overstates

Most of the runtime is about mixing two insulins in one syringe. That is an insulin-specific skill with its own hazards, and it has no application to GLP-1 therapy. Semaglutide and tirzepatide are never drawn into a syringe with insulin or with anything else; the labels for people who also use insulin say to inject them separately, and never to mix. A GLP-1 user should take the single-vial steps and ignore the clear-to-cloudy sequence entirely.

The 45-degree angle is taught here for the syringes and needle lengths nursing students learn with. The instructions for use in the Wegovy and Zepbound pen labels show the needle going straight in at 90 degrees, and most modern short insulin syringe needles (4 to 6 mm) are also used at 90 degrees in adults. Neither angle is wrong in itself; the correct one depends on needle length and body habitus, and the instructions dispensed with your product govern.

Like the VA video, it does not touch the concentration problem. Insulin is standardised at 100 units per millilitre; compounded GLP-1 vials are not standardised at all, so the units on the syringe mean a different number of milligrams for every concentration. The video also does not cover storage, weekly timing or missed doses.
